The challenges and Needs
Medical immobilization (cast, immobilization, etc.) increases the risk of blood clots by over 500 times. In fact, over 50% of all people over 40 get a blood clot when immobilized after a tendon rupture or ankle fracture. But we don't have to go that far; sedentary work and lifestyle also increase the risk of blood clots. Every extra hour of sitting per day increases the risk of blood clots by 10%. "Sitting is the new smoking" according to the World Health Organization.
The calf acts as the body's second heart. With every normal step you take, the calf muscle contracts and pumps the blood upwards. That's why it's so important to try to take at least 10,000 steps each day, preferably spread out throughout the day, to keep the blood flowing. When immobilized or sitting still, the calf muscle pump does not work.
Venous thromboembolism (VTE), often originating as silent deep vein thrombosis in the legs, is one of Europe’s leading causes of preventable hospital-related death, claiming ~540,000 lives annually. Risk is highest during immobilisation and post-operative recovery, yet prevention remains insufficient: anticoagulants carry bleeding risk, Passive measures, such as compression stockings, have limited blood clot prevention effect, or no effect at all on increasing blood flow, poor adherence, and Intermittent pneumatic compression (IPC) devices - external pumps that compress the limbs—are bulky, staff-dependent, and impractical for home use. Beyond acute care, 1.8 billion inactive adults face impaired circulation and rising cardiovascular risk — yet no standard-of-care wearable solution combines automation, adherence and hospital-to-home usability.

CIRCULA®
Circula® is a bioelectronic thromboprophylaxis platform activating the calf muscle pump via targeted NMES. Its patented Smart Search™ technology uses electrode-sensor feedback to identify optimal stimulation points, overcoming key limitations of conventional NMES: manual setup and user discomfort. Lightweight, cable-free and designed for daily use, Circula® enables non-invasive prevention across hospital and home settings.

Circula® is a lightweight, affordable, cable-free bioelectronic thromboprophylaxis platform integrating hardware, smart textiles, and software to restore circulation.
The Control Unit: The hardware "brain" of the system. It coordinates sensing and NMES stimulation using a custom-developed Application-Specific Integrated Circuit (ASIC) which is a customised microchip, designed specifically for the Circula device’s unique requirements, rather than a general-purpose processor. This also minimises the size and energy consumption of the unit.
The Matrix Patch: A flexible 3 × 3 array of nine thermoplastic polyurethane electrodes enabling 36 unique two-electrode stimulation pairings. Adjacent electrodes can be electronically merged into larger “superelectrodes”. This enables personalised stimulation adapted to calf size, tissue impedance, and motor-point location.
The Mobile App: A native Android and iOS application offering real-time monitoring, dynamic data visualisation, and personalised goal setting. It includes engagement games and ensures high-level security through data encryption and GDPR compliance.
The Cloud Platform: An advanced analytics hub that leverages Artificial intelligence (AI) and machine learning (ML) to identify clinically relevant patterns and population-wide trends, providing decision support to optimise health outcomes.
